RFINED PECTORALIS MAJOR MYOCUTANEOUS FLAP FOR HEAD AND NECK RECONSTRUCTION



Abstract
The pectoralis major myocutaneous flap has been used as a versatile and reliable donor flap for reconstruction in the head and neck cancers.
The conventional pectoralis major myocutaneous flap has several limitations
1) The pedicle is bulky. 2) Location of nipple-aleolar complex may be distorted after flap harvesting and donor closure_ 3) The vascular bundle has high chance of kinkng or compression when true island flap is elevated.
In an attempt to overcome these problems, we have modified the pectoralis major flap ; an elliptical cutaneous flap was designed obliquely 4 to 5 cm below the nipple, and the muscle paddle was designed trapezoid shape, and was elevated as a true island flap.
We applied this modification in 6 cases of immediate reconstruction of the defects developed after surgical extirpation of head and neck cancers.
The modified pecoralis major myocutaneous flap provided us: 1) more flexable flap rotation 2) partial preservation of the pectoralis major function 3) aesthetically acceptable donor site 4) safe and reliable transfer of flap.
